To report a submission, you have to click the flag that is inside of the info box right under the submission and beside the description.

If the flag is grey like this, then nobody has reported it yet. If you are sure that it breaks one of the site's rules, you should report it.

If there is a red flag, then there is already one or more reports for the submission. You do not need to report if there is already a report. Reporting something twice does not speed up the procedure, but feel free to provide further proof (link to original) if you're unsure the original reporter has done so.
How do I report a submission?
If you click on the flag, this window will show up:

On this window, you must chose a category for your report.
Here are a few examples:
• Unoriginal: Stolen work
• Remixed Content: Texture packs or skins that have parts copied and pasted from another member's submissions.
• Mistake: Skins posted in the texture packs section
• Spam: Blogs that advertise servers or are filled with copied and pasted text
• Illegal: Distributing the minecraft.jar, or other Illegal downloads
After you have chosen a category you must to explain why you reported the submission. You do not need to write a novel, when reporting something. But make sure that all the information that is needed is included. If it is not clear why the submission has been reported, moderators might ignore the report.
If you report stolen work, always include a link to the original, if possible.
It can be very difficult to properly identify whether or not an item is stolen or not without proof. If a mod has to search for the original first, the stolen work will stay online even longer before it gets deleted (as well as all the other open reports).
What not to do:
• Do not report server because you have been banned etc. PMC has no control over what happens on servers.
• Do not report things just because you do not like the member who uploaded it. All reports are reviewed by a moderator, and if the report does not contain accurate information, it is a waste of their time. This will make them unhappy.
• Do not just post in the comments that the submission is stolen or that you reported it. That does not help the moderation team remove stolen content, and is likely to start a flame-war. Instead, file the report, and then move along.
• Do not write a message to the person who posted the submission. They will never see the actual report text, nor will they know who filed the report. It is only read by site moderators.
If you are not sure if to report or not, report it and explain. Moderators will review the report and make a decision.
