I really do not know how to call it, I am helping my father with the website, he is using OsClass and we want to add a page where users have few options and and when he picks those options it would send the email for example:

User:
Chose this
And this

How could I do that? And also that should be multi options too. For example like
Option 1
Option 2
Option 3
