Best are Optifine, BetterSprinting and Rei's.
Worst is any mod which adds emerald, redstone, lapis or obsidian tools.

Opftifine, without it you'd all die by falling in lava whilst being shot by lag spikes.

best mod:tmi btw the only reason i like tmi more than nei is because 1.tmi is easier to download and install 2. nei dosen't have custom fireworks/enchants/potions if nei had what tmi had i would say nei would be better better

Hmmmmm... in my opinion the best mod out there is Optifine. Although if we were to discard Optifine, I'd say it's Dr.Zhark's Mo' Creatures, NEI, or (For those with a good graphics card) GLSL Shader's Mod (With Sonic Ether's Shaderpack).
The worst mod... I'd say it's X-Ray Mod (I'm sorry, but it's for f@ggots...) or any Hack Client ModPack.

I don't get it.. nei and tmi are the same thing, nei is tmi after 1.8 (that's when creative came out)

NEI is better than TMI because of the recipe function and the compatibility with other mods.

One of the best has to be TMI (Too Many Items). Come to think of it, it is probably one of the most used mods out there since you end up using it all the time. It feels like something Minecraft should have by default!
