What Makes a Roleplay Server Great?

What aspects would you say make a roleplay server great? Is it hardcore roleplay with a unique world and its own lore? NPCs that give quest and using MC mechanics to play Minecraft, but with a story? Is it clear set rules, or a freeform experience?

I definitely think that custom biomes are a big plus in minecraft roleplay servers, and also I suggest having a 100% custom server.
