Hey guys I just programmed this in java. It's a program which you can use to easily get commands for custom potions for command blocks.
Select all of your preferences using the GUI, and then click the command to copy it to your clipboard. Then just head into minecraft and paste it in a command block.
You will need java to run this. I'm sure most of you have java though.

You need java to run Minecraft, so this tool will work for the people who downloaded it.
1. Change the 1st number (373) to the word potion
2. I highly recommend replacing the 2nd number (64) to 1
3. Change to name from "Custom Potion" to whatever you want the name of your potion to be.
Done!
