Hello PMC!
Today I'm bringing you another easy tutorial!
Lets get started!
Step One: Create a (very) fast redstone clock!
This is fairly easy to do!
You just need 2 command blocks, positioned like this:

The bottom command block should say:
setblock ~ ~1 ~ minecraft:air
and the top command blok should say:
setblock ~ ~-1 ~ minecraft:redstone_block
Step 2: Targeting the entity!
To do this you need to give the targeted entity a name
For this tutorial the name was "Animal"
(You can change the name to any of your choice)
Just rename a nametag to "Animal" and name the animal of your choice!
Step 3: Teleporting the animal
To do this only one command is neccesary!
tp @e[name=Animal] @e[name=<Your IGN>]
Replace <Your IGN> with your in game minecraft name! (Rhyming :P)
When done it looks like this:
Step 4: Jump Boost and Resistance
The next step requires to very easy commands, they give the animal Jump Boost 2 and Resistance 1, just so that it doesnt get hurt while jumping!
Command No.1
/effect @e[name=Animal] minecraft:resistance 1000 2
Command No.2
/effect @e[name=Animal] minecraft:jump_boost 1000 2
Alsmost done:
Step 5:Invisibility
All you need to do is a simple in game command
/effect @p minecraft:invisibilty 10000 1
Final Step: Placing a block!
Now all you have to do is place a redstone block!
Right here!
This is the end of the tutorial!
Enjoy your transformation!
