I have been investigating on what is happpening in the music disc 11, and the answer to the question is basically told to us by the person who created the disc, C418, and is simpler than you would have thought.
I have made conclusions in a previous blog about the in-game music disc, found here. I have realized that outside of the game files C418 has made a music clip named "11" or "eleven", the exact same name as the broken disc. Both discs start with a static sound, infering that both discs are cracked to some point. My theory on this is that the in-game music disc is the exact same as the outside music disc, but the one in-game is more damaged, and this explains the random ambience in the background of the in-game music disc. Both music discs, inside and outside the game are both confirmed to be made by C418. Therefore, both music discs are the same because they are both made by the same creator with the same name and the exact same length.
The music disc is the same disc, in-game and outside. Therefore, the music disc is more damaged in the in-game version, because it has more static noise than the outside disc and the item icon shows the disc is nearly destroyed. This only means that the in-game disc was damaged and beaten somehow, and that it used to have the calm piano of the outside disc. The disc in-game is actually the outside disc, but damaged horribly. The walking sounds can be explained as enlargened gaps in the disc, the cough is pretty hard to do, but it could happen, and the tinkering sounds can be explained as the distorted music, same with the ambience in the background. it is pretty clear that the music disc is a distorted version of the outside disc, and this was it all along. Sorry, Herobrine, there's no place for you in this disc.
Therefore, this simple and best-proven theory on the disc is more than likely to be the answer, as the creator of the disc gave it away, and I think i'm the first to bring this theory of the disc up.
