Also, I cannot find any motherboard info on it, so I don't know which models can fit into it. I'd recommend, but not knowing if it would fit or not, I'd rather not even say.

Your processor and the graphics card are the major issue. Can you upgrade both of them with something good in a $150 budget? I highly doubt it. 4GB Ram should be okay just for Minecraft btw.

You need a new power supply, GPU, and CPU. You have components that are just above laptop grade =p

I'd recommend the 750 ti.
Alot of the forums would disagree with me, but if you have a look on my channel , I did some benchmarks on the 750 ti and it works perfectly.
I also have a really old AMD APU that is bottlenecking the 750 ti so you might expect more performance with the CPU you have.

What do I upgrade, I dont know the first thing about graphics cards. My whole entire life, I've used APUS. I dont even know if my motherboard suports one.
You currently have the 'AMD Radeon HD 7310' graphics, maybe look at some benchmarks for some GPU's you might be interested in, they're one of the most easiest thing's to change in your pc.
Also you only have a 220W PSU so you might need to upgrade that to upgrade your
graphics.
Im not sure what motherboard you have but you can most likely install a GPU into it.
