Please be more descriptive on what kind of server this is. I only see a forum. With all honesty nobody will really fill that out without any information given. Is it a survival server? Whats your goal by obtaining staff members. Also this application seems to be copied and pasted from somewhere else "high volume?" assuming you only have 1 or 2 applications that should take about 15 minutes.
