Hello, I am looking for some kind of quiz application/html/php or something for set of questions, and after you answer everything right you will be added on whitelist on my server. I am running this from some application, where I generated quiz and every completition (no matter if all right or not) I got on mail and after I finally got email with all answers right, I manually added that person. Now I am looking for upgrade, but don't know what to do, or how.

What I want from new quiz:
1) input for ingame name/nickname
2) set of 10 questions with 3 options for answers (only 1 asnwer is right - pick one)
3) if you have one or more mistakes in test you have to try it again (unlimited tries)
4) if you have everything right (10 of 10) send email with your nick (maybe email for respose after I add the player)

Optional:
1) Verification if nick is original copy of MC (server only for original - not warez)
2) 30 questions in total (or so), but every time you start quiz you will get 10 questions randomly picket form set
3) option for sending nickname and UUID
4) connection with server for auto add player (UUID) on servers whitelist

Looking forward for any tips/help.
