It's a glitch when spawning one of the skellie skulls with /give (user) (id) or hacks.

Yeah I found it while i was looking for the command block! I forgot what the command block's id was and we used up all our internet

I'm pretty sure it's just one of those items that fill in an un-used ID, and this one just places a skull.
Like glitch pokemon in Pokemon that fill in un-used IDs.
