Disconnected from game

When I play Minecraft servers and realms I am sometimes disconnected from the game. It says: Internal Exception: java.io.IOException: An existing connection was forcibly closed by the remote host. This happens often, does anyone know why this happens and how to fix it? Thanks

Seems like an issue with your network cards/drivers. But it can be also caused by the router/WLAN or even the firewall.

What OS are you using?
Do you get randomly disconnected from the internet?
Have you tried totally resetting MC?
Does this happen on all servers for you?
