I told you it can't be fixed a thousand times, the only solution is to upgrade.
You need to start listening. I am a shader dev, if a shader dev says shaders cannot run on your hardware then shaders can't run on your hardware. You do not need more information, you got the information you need. If that's not good enough for you, too bad. There is no alternative other than to just not use shaders, or use only "lagless" shaders. I have seriously done all I can to help you both in pm and on this thread and all you do is come back asking for more information on how to fix it when there is no other way than to upgrade.
This will probably be the last thing I say on this subject and I recommend nobody else try to help, because the fact of the matter is he's running on an extremely out of date machine and saying anything else may mislead him into thinking there is an easy solution to this that isn't what I just said.

Ok so yeah it is your hardware. It's not supposed to be good looking, all it is is a single file that outputs the original image. No effects, just default Minecraft. You're probably experiencing something similar to a TDR.

Is this the whole log before your Minecraft crashes? If so I do think your computer is not good enough to run shaders. I know they once worked before, but sometimes driver updates will do that.

Alright so what I'm seeing is that the shaders mod gets to the gbuffers then crashes. A shader pack works by using the gbuffers to send information to the composite files then the final file displays the final image on the screen. If you'd like, I could create a very simple shader and see if it works then. If it doesn't then it could actually be the shaders mod in this case. I'll link you the download for the pack in dms, let me know what happens when you try it out.

Your computer might not be able to handle it do you know what specs it is.Usally anything below 6gb might not be able to handle it. Also your shader pack might not be working properly test that by downloading another one and that will tell you whether your computer isn't good enough or just the shaderpack is broke.

You still haven't given us your CPU, and we already have the GPU. Also I would not recommend downloading packs of shaders. Despite not using ad links on my shader downloads, lots of developers do and they could use the revenue from it. You should try to download from official links.
EDIT: I have also stated before that your hardware is extremely outdated, and you may just not be able to run shaders at all anymore. I'm guessing a driver update caused all this to happen, and the best thing to do would be to get a new PC. You may not like the answer, but it's probably the most likely one. Especially since you're still not giving us any information that we need.

Well where can I can find my CPU, and Shaders worked before.

Same way you found your GPU, through device manager Instead of finding display adapters in it though, look for Processors and tell me what is under it. And yes, I know this, and I addressed it saying that I think it was a driver update.

In terms of specs it would be helpful to know your GPU( Graphics card) mainly.
Also which shader(s) are you trying to run? SEUS (for example (like most)) is only compatible with AMD/Nvidia cards.

The shaders mod has moved to Optifine mostly, which can use Forge of course but more often than not people tend to use the Optifine/Shaders standalone because of how easy it is. Reinstalling the shaders mod / Optifine / Forge may not be a bad idea, though. I wouldn't do that quite yet though, because it's still possible that the problem is stemming from his hardware.
EDIT: Did some digging in that log there, his graphics is AMD Radeon HD 6530D and just for a little perspective, that's a $40 card. It has 512MB (not much), a whopping clock speed of 443 MHz, and his is running OpenGL 4.1. The card I develop on is a GTX 660 OEM with 1.5GB and uses OpenGL 4.5. What I'm guessing is happening at this point is it's running out of memory and crashing. I'll try to simulate that real quick and see if I get a similar log just to make sure though.

I'm probably going to guess your hardware is incompatible with shaders or it's just not powerful enough. You seriously need to tell us your specs if you actually want us to answer your questions.
Also, if you could tell us which shaderpacks are doing this that would be nice. You can't just say all, because I'm 100% sure you don't have every shaderpack.
