First you have to make a .zip file by using your world that you created. To do that follow this steps. 0
01.locate your Minecraft folder (Use Run by pressing Windows key and R key on your key board and type %appdata% there. Then you will see a folder named .Minecraft . That is your minecraft folder. )
02. open the .minecraft folder.
03.Find the saves folder ( there is a folder named saves)
04. open the saves folder.
05. there will be a folder with the name of your world that you made.
06. Right click on that. click on "send to" then click on "compressed (zipped) folder".
07.Name it what ever you want.
08.log in to your planet minecraft account.
09.then click on content. ( its at top right corner )
10.click on create new.
11.click on Map.
12. Then fill all the things in that form and add images in to the "add images gallery"
13.to add the world you created click on "upload map file" then click on choose file.
14.then locate the .zip file you created earlier. then click on "open"
15.Finally click on publish live.
