(Notice: This pack is untested below 1.16, but should work all the way down to 1.13)
I finally did it I have modular Teleport Pads working! So here is how this works you get some gold, redstone, different dyes, 2 droppers and 2 eyes of ender to make this system.
Should work in multiplayer

Once you have crafted warp core and 8 teleport crystals(color doesn't matter, but you want to create a unique "code")
After using a pad it will give a 30 second cooldown. Walking 5 blocks away from all teleport pads near you will change the cooldown to 1/2 second.
To remove a teleport pad simply remove the Warp Core and it will deconstruct itself.

Steps to make a working Teleport Pad:
1.) Place a dropper and drop a "Warp Core" on to it. This will create a disabled Teleport Pad.
2.) Place 8 Teleport Crystals, 1 in each of the remaining slots of the dropper to give the pad it's teleport key code. Once you have done this the particles will turn blue and purple.
3.) Repeat step #2 to create a second pad somewhere else in the world. Make sure the crystals are in matching slots.
4.) Standing on linked pads will teleport you between them then give a Cooldown indicated by red, orange, yellow and finally green(0 CD left) particles.

These are for if you can't find nearby teleport pads, craft the item and drop it to locate nearby teleport pads.
Higher ranked locaters find more teleport pads for a longer time.
All of the following recipes can be done in any order in a 2x2 crafting grid:
Rank 1 Locater: Iron Ingot + Ender Pearl
Rank 2 Locater: Gold Ingot + Ender Pearl
Rank 3 Locater: Diamond + Ender Pearl
Rank 4 Locater: Netherite Scrap + Ender Pearl

If you don't want the sounds do /trigger SPTP_portalSound and that will disable it, do it again to enable it.
Particles can be toggled using /trigger SPTP_particles
To safely disable and remove the datapack do /function sp_tp_pads:uninstall
