Need Help Finding a Map

I can't seem to find the map I used to play. It was called SkyBlock 3.0/3.1.
It had:
-Main Island
-Desert island
-End portal island
-nether island
-End island.
So 3 lands in the Overworld. I just can't find it anywhere. Please post a link here or PM me if you know where it is please.
