Finding the small details:
-The dark spot on his leg his infarction
-In his hand is Vicodin pills
-When removing the hat layer you'll see the inside of the Vicodin bottle

You use a ton of banding and pillow shading all over the skin. Banding is when different clusters of pixels line up. (a cluster is a connecting area of pixels that are the same color.) Banding is bad, since it's very noticeable and ugly. Since we're on such a low resolution, it's easier to notice.

On the right side, I outlined where the bands are. On the left, I made an edit to remove the bands, and make the shading more interesting, using your colors. The banding exists all over the skin, in some places not as noticeable as others. It's okay to have them sometimes, as they can't be totally avoided, but we can try to minimize them.

The other thing is pillow shading. Your lighting is pointed towards the camera in every angle you look at. Up to the left or right is a better option. On my edit, the shading is different on each side, to demonstrate better lighting.

Just a few quick opinions, hope you take 'em into consideration.

Can you tell me what goes into designing an angle to shade? Hehe, this was very helpful, my eyes have been opened!
