Zombie Form (ゾンビフォーム) is a form of Kamen Rider Geats.

Parts granted by the Zombie Buckle:

• Zombie Geats Head (ゾンビギーツァヘッド): The head.
• Orange Geats Eye (オレンジギーツァアイ): The orange compound eyes. They have enhanced infrared ability to detect differences in temperature, allowing Buffa to find hidden prey.
• Zombie Crusher (ゾンビクラッシャー): The lower jaw armor. It has an integrated voice chat function to allow clear communication in intense situations.
• Zombie Head Guard (ゾンビヘッドガード): The purple portions of the lower jaw armor. It grants resistances to damage and poison by extending the abilities granted by the Zombie Buckle to the entire head.
• Undead Chester (アンデッドチェスター): The chest armor. It reduces damage and increases combat effectiveness.
• Zombie Spike SA (ゾンビスパイクSA): The shoulder armor. The sturdy armor that specializes in close combat has a spike that can inject poison to create swarms of zombies.
• Poison Chamber Arm (ポイズンチェンバーアーム): The arm armor. They are equipped with the ability to generate poison called "poi-zom" that can zombify living things by relaxing their muscles and ceasing brain functions. The poison can also dissolve objects that comes into contact with it.
• Berserklaw (バーサークロー): The claw equipped on the left hand. It can cut through the enemy and while punching them. It can also poison enemies.

The Zombie Buckle (ゾンビバックル) is a Raise Buckle that allows the user to access Zombie Form and its armaments, increasing their damage and poison resistances. It also allows the user to summon the weapon Zombie Breaker (ゾンビブレイカー).

Parts of Zombie Buckle and their functions (Zombie Buckle information at TV-Asahi):

• Waking Key (ウェイキングキー): The key that can be twisted to activate various commands.
• Boned Gate (ボーンドゲート): After inserting into the Desire Driver and receiving a command from the Waking Key, the gate opens up and releases the Invade Hand.
• Zombie Reactor (ゾンビリアクター): The display that outputs the status of the internal Torus Reactor of the Desire Driver.
• Invade Hand (インベードハンド): A hand that is released from the Boned Gate that interferes with the Torus Reactor to increase output.

The Zombie Breaker is a chainsaw weapon exclusive to those using the Zombie Buckle.

Parts of Zombie Breaker and their functions (Zombie Breaker information at TV-Asahi):

• Break Grip (ブレイクグリップ): The grip. It intakes poison generated by Zombie Form and sends it to the Dock Cylinder.
• Input Trigger (インプットリガー): The trigger to activate special attacks.
• Terrible Chain (テリブルチェーン): The chainsaw. The high-speed rotation gives it enough destructive power to cut through stone. It can also inject the poison that can zombify living things and dissolve objects called "poi-zom" into the wound.
• Deadly Pump (デッドリーポンプ): The foregrip. It uses the pressure within the pump to increase the rotation of the Terrible Chain while supplying poison.
• Dock Cylinder (ドックシリンダー): A cylinder that has a special structure to store various poisons.
