This tutorial is obsolete and no longer applies to the minecraft file system.
While talking to a friend on an awesome server, I realized that most people don't know how to correctly create a resource pack, or even know what it does.
So, first, a loose definition.
Resource pack: A compilation of texture and sound files that is used by Minecraft players to edit the look and/or feel of the game while leaving the default textures and sounds untouched.
Here is how to make a resource pack that adds a new "flavor" to the game.
Step 1: Make a new folder.
First, make a new folder on your desktop. Name this what you want to call your pack.
Step 2: Gather Default Textures and Sounds
Open the Start menu and in the search box, type in %APPDATA%
There should be a folder labeled "Roaming" Double-Click on that folder
Look for a folder labeled “.minecraft” Open it up and you should see a bunch of files and folders.
There should be a folder labeled “Assets” Copy and Paste this into the new folder you made in step 1
Now, go into your “versions” folder in “.minecraft”. Look for The version you are using in your Resource pack and open the contained .jar file with either 7zip, WinRAR, or any other archiver.
Copy this assets folder and put it into the folder in step 1. Windows will ask you if you want to merge the folders.
Click yes to everything.
Close the archiver.
Open the folder from step 1
Inside the assets folder, drag all folders and files into the folder “minecraft”
Delete the Read_Me… files and the pack.mcmeta
Step 3: make pack.mcmeta and pack.png
Go back to the main folder and Right-click on an empty space in the folder.
A drop-down menu should appear. Click on new text document.
Open the document and paste in:

(You can change the description all you like. This is just an example)
Now, open your favorite graphics editing tool (paint, Photoshop, etc.). I Prefer GIMP.
Make a new image that is 128x128 pixels. This will be the icon you see in the resource pack menu.
You can make it look like anything you wish.
Step 4: FUN
You can edit anything and everything to your heart’s desire in this pack
