Quartz Ore: 8 Netherrack around 1 quartz
For 5 Leaves: 2 Lime dye, 1 Stick, and Sapling of which tree you want

For 5 Wither Roses: See Image
Dead Bush: See Image
Music Disc: 4 Black Wool and
