Need of a youtube PARTNER banner

I recently have been added to the banner layout where the banner is above the video on you're channel.
What i want on it.
My skin being blown up by a creeper on the banner with Thecalebtv99 in the center with like a 3D type thing where the text pops out
On the top of the banner have a link bar with the following
The magic button
Twitter
MC website
If that is too much do what you can i just need it done
Also if you can give me a picture of just the banner and not the backround as well so i can map it please.
