Ok.....For your server to be successful you shouldn't ban players for everything! That pushes old and potential new players away!
Kicking for most infractions is what people are ok with. Ban them after 3 or more kicks to the player has occured within a reasonable time frame.
Mute the spamming jerks and then kick them if they continue.
Griefing has different meanings to people. Depending on the degree of the griefing (lava falls in around a player's build/base that does not belong there is pretty serious and uneccissary) then set up a prison type area that the a-hole griefer (and other offenders if you want) must work for their bail.
These are just some suggestions after only reading your rules in the description.
