This is Photo Realism, a 16X16 Realistic Resource Pack. Mostly made by Nothingbutbread (NBB) but it was passed to me to update from now on. He just didn't want to continue this amazing work so I asked if I could and he agreed to let me update it and continue it :)

Make the grass more flush with itself, and make it less noticeable to pick out the block edges.

It's not easy to make it photo realistic. This is as close as you can get to it. Like I said before I have to do a lot of editing to what there is here and the other blocks that I haven't done yet. That is why the texture pack is only V0.62 and not V1.0. This is mainly a test version. I'm making it more realistic.
