Ok, let me first start by saying Moderator is not easy to get in most servers. Sure if a child is running a server you might be able to get it easily. However, if you want to be a mod in a server by professionals, these are the steps you will have to take. Also, it will take quiet a while for you to get your desired position, but if you have patience, it's sure to be yours.

Step [1]: Be an active player on the server.
This means playing a lot, being active in chat, and being part of the community. If the server has a website, be sure to be posting threads and helpful tutorials. Being nice and cool never does hurt either, and I would encourage it. Most of all be mature!

Step [2]: Make the server a better place.
You can do this again by participating in the server itself. Help new players out and be the guy that people go to for help. This can be done without mod and it really helps the staff notice you. Be sure you never break any of the rules, this could eliminate your chances entirely or delay your chances of getting mod.

Step [3]: Make a mod application on the server.
Once you have established yourself on the server and you're a pretty popular guy or girl, make a mod application. Make sure it's professional and something that catches the staff's attention. Be mature while filling it out, but make it unique at the same time.

Step [4]: Be Patient!
This process could take several months. Also, once you fill out your application, do not bother the server staff to look at it. Don't say anything to the effect of, "Hey did you look at my mod application?". Doing so makes yourself sound beggy and will lower your chances of getting mod.

Step [5]: Congratulations!
If you are professional, mature, trustworthy, experienced, involved in the community, and you don't start any drama, then congratulations, you should have mod! However, you may not get mod if the server is not looking for anymore mods or you live in a time zone that is not desirable. That's a low chance that would happen.
