I am currently building a middle-age-map for cityofgalaxy.de and this is the raw model of it. It contains about 16.840.000 square-"meters" and is free to use, just contact me! If you want any minor changes, just contact me! :D
There are no custom trees and it is not survival friendly. This is the version without lakes, I also uploaded one with. Have fun exploring it!

Wow this looks very nice.
Especially this swamp like biom (picture 7) I like.
Are you thinking about to add some trees?

I actually already did add forests, but the trees I used come from many different people on this platform and I first want to add the towns and all of that, before I post a final version. I plan on building more banner widow trees and use them for the swamp biom. :D
