This datapack was made for those who want more of a challenge when playing in their Minecraft worlds. Totems of undying are known for being a powerful "get out of jail free" when in a tough situation, especially in hardcore mode. Totems of undying can still be used as a countermeasure to death with this datapack enabled, however you will still have to be careful in any tough situation you encounter, since you can easily die once you've lost your totem!
I haven't tested it in a multiplayer world, however I do believe it would work in a multiplayer world.
