This is a simple concept, but taken a bit too far.
It originally started as a simple 3x3 piston door. 
I took it a bit further, and put a combination lock on it. This led to compact-ness issues, so the middle block remover was taken away.
Then, I realized that it needed a reset mechanism, to prevent multiple people from getting in. 
The reset mechanism could be used to re-open the door, so I added a command block to teleport the player through a wall into a room on the other side.
After showing it to my friends, they said that someone could cheat and watch someone enter the password, so I added an anti-cheat door on the entrance, which only allows one person inside at a time.
I figured that someone could just use the 1-person-only thing, and then jump out immediately, so I put in an anti-troll wall, which blocks the exit for a split second, until the anti-cheat door is either fully open, or fully closed.

Originally, it was made with command blocks saying when a number is activated/deactivated. The reset mechanism would display "*combination* is deactivated*". Therefore, I deleted all the command blocks, and set them to be redstone torches, which would display whether the numbers are toggled or not. This made me have to move a lot of the wires, so there would be no short circuits. 
I then had to make a way to exit the secret room, so I added a command block teleporting exit, to prevent people from running inside as a person leaves. 

This entire project took about two hours.
