Some people can be pretty racist sometimes, so that sucks.
That's not just in mc alone, that's irl, and in other games/internet in general
But yeah, I voted taunting. Bedwars is just the dumbest game because of crap like this. One round this a****le kills me at my base, but he doesn't destroy my bed even though he can. He just lets me respawn and kills me over and over. Since then I haven't touched that minigame.
That's just normal pvp in general.
Have you seen the amount of flame from a game like League of Legends, one of the most toxic games?

You forgot to add "Developers who ditch features like the Firefly"
The software development/business world is run by money
Devs can make promises but it is much more harder to keep it, especially with deadlines and bosses that are greedy (Microsoft)
For example, I know someone whos a developer in a small game studio. He wants to change a small piece of code. The people verifying that code can take from an hr to a day just to verify that code. And this is just changing content. Now go through that processes a few hundred -> thousands of times but with a set deadline of about 6 months -> 1 year. They then have to do stuff like balancing and bug testing which contrubute to that hellish process.
