Well you can use http://minecraft.novaskin.me/ if you want something simple.
blender, mine-imator or Cinema 4D if you want to get more complex.
I've given a few of them a shot when making thumbnails myself.

No, what i mean is like how do they get the mine craft characters to pose and hold stuff?
Likely something like Photoshop, but custom thumbnails can be made with any graphics program like Paint.NET or Gimp.
