The cave noises are really just ambience sounds and you don't have to be in a cave to get them. Not quite sure about the Steve skin one? If the world is a realm between you and your friend, I would check to make sure it's set to invite only. Sometimes players that you have recently played with, whether on a server or a realm, can join your smp's.
