The wither head and arm (light blue section) is offset from the body.
On the arm, there are more pixels on one side than the other side, causing it to be offset. (I've removed the two heads for your convenience)
Look on the back of the wither head. You'll see that the head isn't aligned with the body as well.
What's the reason for all this? The main body (pink) is 3 pixels wide, which is an odd number. But the main arm is 20 pixels wide, which is an even number. Mojang didn't want to have pixels centered with a pixel (In this case 20 pixels perfectly aligned with 3), because it would be "Unminecrafty" and not be that of block textures. But even then they still flipped it up and even messed up the command block, coco bean, chiseled stone brick, and kelp block texture (along with many more block textures).
Someone fix this with your texturing wizardry.
