Player Hunter is a mod that adds in a player hunter that will hunt players and kill them.

The Player Hunter can spawn with a health of 10-1000.

The Player Hunter will hunt you down, he will break blocks to get to you, HE WILL find you and kill you.

What happens on Creative Mode? It will kill me still like a Terminator Steve? Also does this thing spawn in peaceful too?

It can't kill you in creative and It won't spawn in peaceful, This can be changed in the config file once it's added.

I dont like how he can destroy blocks to get to the player and its a good mod i downloaded othe mod noting yet so keep doinh good on ir mods who ever builted this mod and mod maker how do u make mods on pc?

What are the conditions for him to spawn, same as a mobs? Also what is the spawn rate?

He will spawn randomly, there is no set spawn rate.
