I know most people want to have there stuff on the popular list on the home page! As I do also.
First you want to make sure the title is catchy!
For example:
This caught my attention straight away with its interesting title making we what to know more! BUT with skins give it a name like 'Mud Face' or if a real creature/animal just its name 'Elephant'. The reason for this is when people are on PMC and they want a elephant skin they'll search 'Elephant' and not something silly like 'My fat little baby elephant i made today' just doesn't happen... regularly :P
Secondly the picture!
If its a skin then it doesn't matter but when uploading a blog, project, texture pack, mods and servers you want to make sure you get it right!
Texture pack:
Make the first photo clear and of a large terrain area or even better make a show of loads of blocks, here's an example: http://www.planetminecraft.com/texture_pack/gugge-craft-texture-pack/
Blog:
A photo that represents your blog is best or like my Homer with a large space for a title! Here is a nice example: http://www.planetminecraft.com/blog/who-is-your-favorite-from-yogscast/
Mod:
I am no expert when it comes to mods but what gets my attention is a photo of the actual thing the mod does, here is an example: http://www.planetminecraft.com/mod/whocraft-18---the-doctor-whominecraft-experience/
Projects:
Make the main photo the most updated of the whole project! Also add text, this will make it lots more awesome. Here's an example (My first project): http://www.planetminecraft.com/project/roman-colosseum/
Servers:
Make the main photo something that is amazing in the server and add text! Either by making the servers name with blocks or using a photo editing software! Here's an example: http://www.planetminecraft.com/server/the-land-of-gazamo-bukkit-permissions/
To personalise my pictures I use Picnik It is really easy to use and has loads of features! Here's an example of one of mine: http://www.planetminecraft.com/blog/lukyzades-amzo-skin-shop/
And the important bit! The content (The title and picture is important to get people to even look at the content so keep that in mind.)

Also enlarge the font to make it look easier to read and not as bare I also use bold (Personal choice.) Don't make your text dense it looks boring and hard to read and just overwhelms the reader!
