To upgrade your wooden, stone tools to gold tools you will need:
1x Gold nugget

To upgrade your leather and chainmail armor you will need:
1x Gold nugget
.
To upgrade your iron tools and armor you will need:
1x Diamond
.
Process of upgrading:
Place the Tool/Armor you want to upgrade in a smithing table in the first slot and iron nugget, gold nugget, a diamond in the second slot to upgrade them.

Note: If Your Tools Have Enchantment They Would Not Be Removed When You Upgrade Them.
