If You're Talking About a Limited Amount Of Uses On The Totem Then You Would Have To Use a Custom NBT Tag Like {CustomData:{Uses:<Value>}} And Make /item replace Commands For Every Number Or Use a Mod To Allow Editing Player NBT Data And Use Scoreboards To Store And Modify The Uses On The Totem, If You Don't Want Mods And Make a ~1000 Use Totem, It Would Be Almost Impossible To Write Those Commands Without a Generator Or Script To Write Them.

If You're Talking About a Death Limit Like Hardcore Mode Then Just Make a Scoreboard That Tracks The Amount Of Deaths The Player Has And Another "dummy" Scoreboard That Stores The Max Value, Then Just Check If The Deaths Value Is Higher Or Matches The Max Value And If The Check Succeeds Then Set The Player Into Spectator Mode (Or Run Whatever Commands You Want)
