I thought it wasn't quite clear; do you only lose a life if a player kills you or just if you die normally. Secondly, what happenes after you lose your last heart?

yes if you die from a creeper or fall damage (etc) you lose a heart. when you lose all your hearts you're either banned or put in spectator.

Could you make one with out the 20 hearts cap however this is amazing
