I got bored today, and I've had this idea on the backburner for quite a long time, so I decided to quickly try to whip it up and see how it went!
I made it in only a few minutes and the goal was to just annoy Speedrunners by giving them incredibly difficult to deal with handicaps simply by trying to speedrun the game. Each parasite gives the player 2 negative potion effects.
I won't tell you how you get the parasites, that you will have to figure out yourself, but I will tell you how to get rid of them because I'm nice.
Wide Parasite - Jump in lava to burn it off, it is quite resistant to physical attacks, but it isn't at all heat resistant
Long Parasite - Sit in Powder Snow until it begins to hurt to freeze it off, they really hate water but it doesn't hurt as much in its liquid state
Bloated Tick - Suffocate yourself with Gravel/Sand

Tapeworm
Dies to fall damage
Removes saturation while alive.
Can be obtained by breaking stone on dirt or grass, and if you eat something with tapeworm eggs, you get the parasite.
Leach
Suffocates out of water.
Slowly takes away health, but can't kill.
Small chance to get the leach while swimming.
