Custom Pause Screen?

I literally can not seem to find anything anywhere on how to design a custom pause screen...I know it's possible due to seeing it on a server, but I can't seem to figure out how to do it myself, could anyone help?

I don't think you can change the pause screen layout, but you can change how the buttons look and the text on them.
The Buttons are in textures/gui/widgets.png and the texts can be adjusted in a language file.
Otherwise, there is a optifine feature allowing you to add dimension specific custom loading screens.
