HELP NEEDED!!

I've been making a texture pack and I want a picture as the pack png. I have made sure it is 128x128 pixels but when I go into Minecraft all I see is the default picture. Does anyone know what has happened? If so can you please please tell me.

Make sure the image is names pack.png, make sure that your in 1.3.1.-2. make sure also its not zipped incorrectly.
are you sure you put it in the texture pack folder gotta check everything
