So I was thinking about making a second texture pack as my first one didn't turn out great. So I really don't actually know anything about making a texture pack so I thought I'd ask how to make it so if you rename an item it changes the texture. Is this possible for block textures too? Do you need Optifine?

sadly, custom blocks ine the sence of renaming isnt possible, even with optifine, but what you can do is utilize stuff like double slab textures (even possible without optifine), biome dependent textures or use differenct kinds of ctm to connect the block to different textures, best for this would be a vertical+horizontal as well as a repeat ctm/ full ctm replacing the vertical one, so you can have on one block: single texture, horizontal texture, vertical ctm texture.
