you have to go to your minecraft.jar file and find any entrys with the word TMi and delete them
No, because there is also the ft.class file and that replaced the original, so if you don't have the original, you will get the black screen
I don't think there is a way to uninstall it..
You will probably have to force update and get a fresh minecraft.jar and then reinstall any other mods you had
