I just started playing Minecraft again after not touching it in ages, and I encountered a glitch where in creative mode my body would disappear after I flown past a certain height, and I found out that the same thing happens to mobs.

I found out that Optifine is the cause of the problem HOWEVER, I can't uninstall optifine because without it Minecraft can barely manage 20-25 frames and I really prefer to be running at least 50 frames average. (Skyrim runs better with medium-ish settings for christ's sake)
I want to get this fixed so it won't cause any problems for me when I'm playing on PVP servers, so any help I could get would be greatly appreciated.

This has never happened to me before at all. So I don't know what to say.
Umm Tell me your version of optifine and what MC version you're using.
Maybe then I can tinker around a bit and see if I get the same problem?

Try unistalling and reinstalling optifine.
