I am warning everyone that GermanLetsPlay hacked my server, and has 21 alt accounts. I didn't take a screenshot of the alts, but if you have McBans Premium you can /altlookup GermanLetsPlay and find out.
He destroyed spawn, and much more. He somehow gave himself Admin or OP and banned everyone online at the time.

It could have been the authentication exploit that was recently patched. Update your server and you should be fine. If you want to prevent future authentication exploits, and from session stealers, you can use sk89q's method of host keys with worldguard.
Another method would be to find a plugin which limits staff accounts to a certain IP.
