How to make huge floating items?

I saw this in a server recently and was wondering if anyone knows how to make these?

They're most likely using a giant with an item on its head.
I dont remember if the item actually renders or not though.
The only other way I can think of is giant custom 2d entity models, they then put on an armorstand or another entity.
