Item disappears when players place them!

When players place a block it disappears from the inventory and when they click again it re-appears. I use modifyworld to stop block placement it caused this. I also tried with worldguard and deleted modifyworld and it did the same. I have no other plugins which could interfere. PLEASE HELP!
