If it is Minecraft mobile version, you can Minecraft Mod Apk to fix the crash
Apologies, I didn't have much time yesterday to look at this pack.
Well, there are a few problems with getting this to work. The first issue is that you have included the 'Hospital_Mod_Facilities_Pack' on its own. This is an add-on pack that requires the Hospital mod which you can find here : https://www.curseforge.com/minecraft/mc-mods/the-hospital-mod/files
This gives us a little headache. If you really wish to include this mod, then the highest Forge version you can use is ver 14.23.5.2768.
The next issue is that you've included the 'Chisel Mod'. This mod seems to be missing its required dependency, the 'Connected Texture Mod' which you can find here : https://www.curseforge.com/minecraft/mc-mods/ctm
Because we are using an older Forge, the highest version of Chisel you can use is ver. Chisel-MC1.12.2-0.2.1.35, and the highest version of the Connected Texture mod you can use is ver. CTM-MC1.12.2-0.3.3.22.
The next issue is that you appear to have 2 versions of the Advanced Modern Warfare mod in the pack :

I'm going to assume that you do wish to use the Hospital mod (there are quite a few add-on mods you can use with this : https://www.curseforge.com/minecraft/mc-mods/search?search=Hospital ), in which case, rather than going through each mod version you need to use with Forge ver 14.23.5.2768, I've put a working pack together with Jei and Hwyla, just in case you hadn't got around to including them yet. Click the link on the Spoiler below to reveal the versions of the mods I put together which will load and start a new map - no guarantees about how stable it'll be though - seems fine, but you never know.
Well, there are a few problems with getting this to work. The first issue is that you have included the 'Hospital_Mod_Facilities_Pack' on its own. This is an add-on pack that requires the Hospital mod which you can find here : https://www.curseforge.com/minecraft/mc-mods/the-hospital-mod/files
This gives us a little headache. If you really wish to include this mod, then the highest Forge version you can use is ver 14.23.5.2768.
The next issue is that you've included the 'Chisel Mod'. This mod seems to be missing its required dependency, the 'Connected Texture Mod' which you can find here : https://www.curseforge.com/minecraft/mc-mods/ctm
Because we are using an older Forge, the highest version of Chisel you can use is ver. Chisel-MC1.12.2-0.2.1.35, and the highest version of the Connected Texture mod you can use is ver. CTM-MC1.12.2-0.3.3.22.
The next issue is that you appear to have 2 versions of the Advanced Modern Warfare mod in the pack :
mw_2.0-0.4.4.6_mc1.12.2.jar
mw_2.0-0.4.6.2_mc1.12.2.jar
........ this won't work.
I'm going to assume that you do wish to use the Hospital mod (there are quite a few add-on mods you can use with this : https://www.curseforge.com/minecraft/mc-mods/search?search=Hospital ), in which case, rather than going through each mod version you need to use with Forge ver 14.23.5.2768, I've put a working pack together with Jei and Hwyla, just in case you hadn't got around to including them yet. Click the link on the Spoiler below to reveal the versions of the mods I put together which will load and start a new map - no guarantees about how stable it'll be though - seems fine, but you never know.

This seems to be a problem with minecraft itself. Is your forge updated properly/is the latest version?
