If you're using a texture pack, it could be the texture pack or problems loading it. If you dont use or have a texture pack on when its doing this, it could be mods if you have them.if you dont have any mods, do this:
1) Copy your saves and stuff and paste to your desktop (the ones u need)
2) Re download minecraft's packages
3) Put your saves back
4) fixed. [if not fixed idk then]

I think it is a glitch with optifine, if you're using it. Just set chunk loading to smooth or default, multi core creates bugs like that,
Wow I had the same problem with my moms laptop and what I found out that it is graphic card that is causing that thing to happen.

Do you have any client mods?
It could be your graphics card needs an update or a new one but its your call

disable high DPI functions in windows and set it to %100 normal
It Could be That U have 3D Option on
Either Bad Graphics card or glitchy Version so Re-install the game and if that doesn't work get a new Graphics card
