Mipmapping not working for texturepacks?

How do you get a texturepack to work with Optifine's mipmap feature?
The default texturepack works fine. It gets smoother the farther away the texture is.

I also tried it on Misa's x64 texturepack and it works. I tried it again on a x256 texturepack and it worked there too.

Recently I modded a texturepack, as in used my favorite textures from about 5 different texturepacks and put it into one texturepack. All the textures work but it's not mipmapping.

Is there something special you have to add to get it to work or what?
