Noob needs help *CIT, Blockbench*

Hi so I've been trying to find a tutorial that changes the model of tools, armors and weapons with 3D model but sadly couldn't find any. What I'm trying to achieve is to replace the model without using name change, when crafting an item I want it to automatically changes it into my 3D model. Is it possible?? is there any tutorial for that? THANKS!
