Has anyone ever had their server hacked where a player (or a bunch of) can manually OP themselves?

Are you with a hosting company? If so contact them or move hosts.
i've changed the password twice today and someone keeps managing to repopulate my ops.txt file
never happened to me on this host before and I've had the host for over a year, and I have a non-sensical 32 character alphanumeric + symbols type password.

There is no force OP. So either your host is retarded or you have a bad password.

Console password isn't that easy to figure out
Nope. If you have I would suggest changing your console password.
